Obtaining Your Medical Records
For Those Seeking COVID-19 Test Results
Once you have submitted your request for an email copy of your results, a minimum of 4 days is required for turnaround. Only positive test results will be communicated via telephone Monday through Friday. For more information, please call 951-696-6013, option 2.
The Southwest Healthcare System Health Information Management department maintains records of the care patients receive while they are at Inland Valley Medical Center and Rancho Springs Medical Center. As required by state and federal law, these records are kept in strict confidence and are only released with proper authorization from the patient. We offer the following options to allow patients to obtain medical records and/or radiology studies from Rancho Springs Medical Center or Inland Valley Medical Center:
Online. For the quickest response time, submit your medical record request through our online medical correspondence system.

In-Person. As we continue our efforts to combat the effects of the Coronavirus (COVID-19), the safety of our patients and staff is of upmost concern. Therefore, we are restricting in-person access to our Release of Information suite with the following exceptions noted below. Please use the other options provided on this website to request access to your records, i.e. online, mail, phone, fax, and/or via your provider. We apologize for any inconvenience.
Exceptions for In-Person Requests: Process servers and patients/representatives who require urgent/same day imaging CDs.
The Centralized Release of Information department is located at 25485 Medical Center Drive, Suite 106, Murrieta, CA 92562. The department is open from 8:30 a.m. to 4:30 p.m., Monday through Friday.
Note: While you are at your provider’s office for your follow-up appointment, the provider’s office can call the Release of Information department (951.696.6013 opt. 2) for any required documentation and we will fax the information directly to their office. Also, imaging CDs can be mailed directly to your provider.
